Programme Mentoring Templates

The Programme mentoring documentation and reporting templates are as follows:

The Support and Development Partnership Agreement – identified by the SDPA1 in the top left hand corner. 

This is completed and retained by the graduate and mentor.

The Mentoring Contacts Record template – identified by the SDPA3 in the top left hand corner.

This is completed and retained by the graduate and mentor each time they have an arranged i.e. formal face-to-face mentoring meeting (at least eight formal face-to-face mentoring meetings are required) and on an agreed basis eg. weekly, fortnightly or monthly, for as required (discretionary) phone, text, email or face to face contacts.

The Formal Familiarisation and 3-Way Conference Record template – identified by the SDPA4 in the top left hand corner. 

This is a single form that is completed and retained by the graduate and each time a formal familiarisation or 3-way conference activity takes place, it should be entered on this form.

The Mentoring Summary template – identified by the SDPA5 in the top left hand corner. 

This is forwarded to the MFYP National Coordinator at the end of each quarter (as shown on the template) by both the graduate and mentor.  They use the information from their completed SPDA3's (formal and discretionary mentoring records) to complete this template.
